Social Prescribing Program

Provided by People for a Healthy Community on Gabriola Society (PHC)

Connects seniors to non-clinical services to help improve their health and wellness.
The Social Prescribing Program helps link older adults with Gabriola-based resources to diminish the negative health effects of social isolation and loneliness.

How it works: Gabriola doctors, health practitioners, support workers, and community members identify residents aged 55+ who would benefit from non-medical community-based programs and services; A referral is made to PHC’s Seniors’ Navigator, who then reaches out to the individual to offer support in accessing appropriate local resources and programs that support health and wellness. Services offered range from systems navigation to information about social and recreational programs, healthy nutrition and support for ageing-in-place.

An individual may be eligible if they are:
  • 55 years of age or older
  • Currently living in Gabriola
  • Willing to engage in one-on-one wellness planning and activities

Referral Instructions: Note that a doctor’s referral is not required. Self-referrals are welcome. To make a referral, please fill out the form and email it to seniorsnavigator@phcgabriola.org.
